If you are requesting an increase for one specific employee, please include the name of that employee.

If you are requesting an increase for everyone in a specific title, do NOT include names and skip the next entry.

Name of the individual in the position. Do not include names if you are asking for an across-the-board increase for everyone within a title.

If you are requesting an increase for a specific employee, include the position number and title.

If you are requesting an across-the-board increase for every employee in a title, include all the position numbers and the title.

Position Info. This can be one position or all the positions in a title if you are asking for an across-the-board increase.

In this space, provide the justification for an increase. It is helpful if your justification includes: permanent job duties of critical importance, a greater scope and impact compared to previous functions, there is significant pay compression between the employee and subordinate staff, or a pay study has determined the need to reclassify this position, the comparisons of internal positions that may require an adjustment, etc.
